This directive:

The Chicago Police Memorial Foundation is a nonprofit charitable organization. The intent of the foundation is to “Never Forget” the sacrifices made for the City of Chicago and its citizens by officers who are killed in the line of duty, suffer catastrophic injuries in the line of duty, or suffer fatal or catastrophic self-inflicted injuries. The foundation will commemorate and provide assistance, as appropriate, to eligible police officers and/or their families.
III.Responsibilities and Procedures
  • A.On behalf of the Superintendent of Police, the Special Activities Section, will act as liaison between the Chicago Police Memorial Foundation and the Department.
  • B.Crime Prevention and Information Center (CPIC) will ensure that an immediate notification is made to the Special Activities Section when a sworn member:
    • killed in the line of duty;
    • 2.dies as a result of injuries received in the line of duty;
    • 3.suffers a serious injury requiring hospitalization while in the line of duty; or
    • 4.suffers fatal or serious self-inflicted injuries.
  • C.The Special Activities Section, will ensure:
    • 1.immediate notification of the appropriate representative of the Chicago Police Memorial Foundation in all instances listed in section III-B of this directive.
    • 2.proper coordination between the foundation and the Chicago Police Department.
  • D.The commanding officer of the deceased or hospitalized member will assist Special Activities Section personnel in compiling all needed information.
