I.Purpose
This notice informs Department members of their legal responsibility to file:
  • A.a STATEMENT OF ECONOMIC INTERESTS as required by the Illinois Governmental Ethics Act (Chapter 5, Illinois Compiled Statutes, Section 420/4A -101).
  • B.a STATEMENT OF FINANCIAL INTERESTS as required by the Governmental Ethics Ordinance (Municipal Code of Chicago, Title 2, Chapter 2-156-150).
II.Illinois Governmental Ethics Act
  • A.Department members who are required to file a STATEMENT OF ECONOMIC INTERESTS will be determined by their position title code, and will receive notification from the Cook County Clerk’s Office, via United States mail, annually on or before 1 April.
  • B.The STATEMENT OF ECONOMIC INTERESTS form must be completed and returned to the Cook County Clerk’s Office, by 1 May of each year.
  • C.Members may direct inquires to the  Office of the County Clerk .
III.Governmental Ethics Ordinance
  • A.Department members who are compensated at a yearly rate, determined by the Board of Ethics, are required to file a STATEMENT OF FINANCIAL INTERESTS annually.
  • B.Members who are required to file, based on their Department salary only, will receive a STATEMENT OF FINANCIAL INTERESTS form from the City of Chicago Board of Ethics, via United States mail, annually on or before 1 March.
  • C.The STATEMENT OF FINANCIAL INTERESTS form must be completed and returned to the City of Chicago Board of Ethics, by 1 May of each year.
  • D.Members who are required to file, but who do not receive a form in the mail, will contact the City of Chicago Board of Ethics to obtain a form.
  • E.In accordance with MCC 2-156-150(f), the Governmental Ethics Ordinance, no employee shall be allowed to continue their duties or receive compensation from the City, unless they have filed a Statement of Financial Interests, as required by the ordinance.
  • F.Members may direct inquiries to the  City of Chicago Board of Ethics .
