Sobriety Safety Check File

I.Purpose
This directive specifies responsibilities and procedures for completing the Sobriety Safety Check File.
II.General Information
  • A.The Sobriety Safety Check File contains all reports generated concerning a specific Sobriety Safety Check.
  • B.Members will refer to the parent order of this directive for procedures concerning the preparation, maintenance, and forwarding of the Sobriety Safety Check File.
III.Responsibilities
  • A.The officer in charge will ensure the following documentation is included in the Sobriety Safety Check File and forwarded to the district review officer with the original General Offense Case Report (CPD-11.380) denoting the noncriminal RD number:
    • 1.Originals of the:
      • a.Sobriety Safety Check Point Log (CPD-21.640);
      • b.Sobriety Safety Check Activity Reports (CPD-21.641);
      • c.Sobriety Safety Check Results Log (CPD-21.642);
      • d.Sobriety Safety Check Site Selection Authorization For Private Property form (CPD-21.643), if used;
      • e.Sobriety Safety Check Pre-Event Notification form (CPD-21.644);
      • f.Sobriety Safety Check Post-Event Notification form (CPD-21.645);
      • g.detail order prepared by the officer in charge and signed by the district commander;
      • h.Sobriety Safety Check approval from the Chief of Patrol.
    • 2.Copies of:
      • a.all case, arrest, and related reports and documents resulting from and related to the Sobriety Safety Check;
      • b.notifications to the Department of Streets and Sanitation;
      • c.any news articles related to the event.
  • B.The command staff member of the Office of News Affairs will, using the Sobriety Safety Check Post-Event News Release form (CPD-21.647), ensure a copy of the post-event news release is immediately forwarded to the:
    • 1.Records Services Division for inclusion into the original Sobriety Safety Check File for the specific Sobriety Safety Check.
    • 2.requesting district for inclusion into the district copy of the corresponding Sobriety Safety Check File.
    • 3.Traffic Court / Records Unit for inclusion into the Traffic Court copy of the corresponding Sobriety Safety Check File.
  • C.The command staff member of the Research and Development Division will, using the Sobriety Safety Check Post-Event Internet Posting form (CPD-21.646), ensure a copy of the post-event internet posting is immediately forwarded to the:
    • 1.Records Services Division for inclusion into the original Sobriety Safety Check File for the specific Sobriety Safety Check.
    • 2.requesting district for inclusion into the district copy of the corresponding Sobriety Safety Check File.
    • 3.Traffic Court / Records Unit for inclusion into the Traffic Court copy of the corresponding Sobriety Safety Check File.
