Elevator, Escalator and Amusement Ride Accidents

I.Purpose
This directive continues procedures to be followed when accidents involving elevators, escalators or amusement rides are brought to the attention of the Department.
II.Reporting Procedures
  • A.A member investigating an accident involving an elevator, escalator or amusement ride will:
    • 1.prepare the appropriate case report, indicating that an extra copy be forwarded to the  Bureau of Elevator Inspection, Department of Buildings .
    • 2.inform the Office of Emergency Management and Communications (OEMC) of the circumstances.
  • B.The OEMC watch commander will:
    • 1.notify the  Bureau of Elevator Inspection, Department of Buildings
    • 2.ensure that the dispatcher informs the member assigned to the accident investigation of the name of the inspector who was notified so that this information may be noted in the narrative portion of the case report.
