I.Purpose
This directive:
  • A.informs Department members of an amendment to the Liquor Control Act, effective 7 January 1993.
  • B.introduces and establishes procedures for the use of the Law Enforcement Report of Conviction form (CPD-11.441) required by the Secretary of State.
  • C.provides a reproduction of the form on the reverse side of this directive.
II.Relevant Statute

Public Act 87-1253, the Liquor Control Act, Illinois Compiled Statutes Chapter 235, paragraph 5/6-16
III.Responsibilities
  • A.When an arrest for a violation of the Liquor Control Act, as stated in Item II of this directive, has been effected, the arresting officer will:
    • 1.complete the boxed-in portion of the Law Enforcement Report of Conviction form and record the Arrest Date on the form; and
    • 2.present the completed form to the desk sergeant at the detention facility.
  • B.Desk personnel preparing court papers will ensure the Law Enforcement Report of Conviction form is attached to the Court Sergeant Copy (blue) of the Arrest Report (CPD-11.420)
  • C.Upon conviction of an individual for a violation of the Liquor Control Act as stated in Item II of this directive, the court sergeant / court officer assigned to the court hearing the case will:
    • 1.record the court information on the Law Enforcement Report of Conviction form;
    • 2.sign the form and enter his star number, agency name, address, telephone number and date the form was mailed to the Secretary of State;
    • 3.place the completed form in a Department envelope addressed to  Secretary of State, Driver Services Department .
    • 4.forward the envelope to the Document Services Section for mailing to the Secretary of State.
